Permit Service Company
Insurance company name, policy number & the state-issued must be provided in order to obtain a permit. Certificate of coverage must be on file with the Department showing a minimum bodily injury of $300,000 for each person, $1 million for multiple persons in any one occurrence and $1 million minimum for property damage.
